Putting a 350 in my '91 and found a NV3500 from a C1500 on ebay, just wondering if it would fit with no crazy modifications. Don't know much about transmissions, are all GM 3500's the same dimensions?

No they are not the same.
Behind a 350 in a light truck you'll find yourself starting in 2nd because the full size truck has a lower first gear that will wind out real quick.
Wide ratio - NV3500 GM 1500
1st-4.02 : 1​

0.73 : 1​

Close ratio - NV3500 GM S-10
1st-3.49 : 1​
